Complete your Application
Complete the Admissions Application and submit proof of government-issued identification, your high school diploma or its equivalent, Social Security number (Tax ID number if no Social Security number), a copy of professional licensure if applicable, and a completed Student Health Form signed by your physician

- High school diploma or its equivalent
- Social security number (Tax ID if no SSN)
- A copy of professional licensure if applicable
If you do not have your high school diploma, you may have your school send an official transcript to:
